Blank logs marketed as Physician's Daily Record, published by the Kersten Publishing Co. of Fort Dodge, Iowa with manuscript ledger entries throughout the text containing information in three fields including patient name, services rendered and charges incurred. Procedures often recorded include: vision tests, glucose monitoring, and, routine examinations, of children (including the polio vaccine) and adults. Charges generally range from $2.00 to $5.00 per visit and many patients appear to keep credit accounts. Monthly totals of gross receipts, credit accounts and payroll are also entered.
